Pre-Primary to Year 6 Application for Enrolment

Please complete the above application form (including EALD form) and collate the following supporting documents:

  • Child's Birth Certificate(original or certified copy)
  • Current Australian Immunisation history statement (required for Kindy and highly encouraged for other Year groups)
  • Proof of residential address: current rates notice or current rental agreement
  • Current utilities bill dated within the last 2 months
  • If child or parents were born outside of Australia: proof of Australian citizenship or permanent residency: copies of overseas passport and visa
  • If applicable, Family Court Orders or parenting plans registered with the Family Court
  • Medical condition requiring possible action at school (e.g. asthma or severe or moderate allergy): please supply the most current Medical Action Plan signed by a medical practitioner
  • Diagnosed learning difficulty or disability: please supply any relevant information such as the diagnosis report
  • Latest school report (Pre-Primary to Year 6)

In-Boundary Enrolments

The principal will enrol an eligible child whose usual place of residence is within the local intake area for the school. Please see link below showing the map with the local intake area for Dianella Heights Primary School.

Out-Of-Boundary Enrolments

While all enrolment applications are considered, we cannot guarantee placements for students who live outside of our local intake area. If you make an out-of-area application and that year group is at full capacity, your child will be placed on a waitlist, and you will be notified if a place becomes available. All out-of-area enrolments must be approved by the principal before proceeding.

All applications are assessed in line with Department of Education guidelines. In some cases, the school may receive more applications than places available. When this happens, the principal prioritises applications.

    They have a sister or brother enrolled in the same school next year.
    You live nearest to the school.
  • Second priority You and your child live in the school’s local-intake area.
    They do not have a sister or brother enrolled in the same school next year.
    You live nearest to the school.
  • Third priority You and your child live outside the local-intake area.
    They have a sister or brother enrolled in the same school next year.
    You live nearest to the school.
  • Fourth priority You and your child live outside the local-intake area.
    They do not have a sister or brother enrolled in the same school next year.
    You live nearest to the school.

Important

A substantial portion of our school’s funding depends on accurate information collected at enrolment regarding a child’s language spoken at home. If your family have a language background other than English or speak a language other than English in the home, please ensure you complete the EALD profile sheet at the bottom of the Application Form.
